After Lori died Rick went a bit insane. He got phone calls that he thought were other survivors, but were in fact voices of the people who have died. Including Lori. Not to mention the fact the phone was never actually plugged in. He's basically going down a road of insanity, and the person at the end of the last episode was supposedly Lori(I didn't think it looked like her).
